She was predeceased by: her parents, Clifford Cook and Anna Cook (Rubbo). She is survived by: her husband Randy Shaffer; her siblings, Charles Cook (Gladys), Clifford Cook (Janet) and Cheryl Maier (Tim); her nieces, Cassandra Fetty (Jason), Callie Krueger (Irvin), Kayla Thompson (Robert) and Gina Maier; and her nephew Rick Maier (Melissa).
The family suggests, in lieu of flowers memorial contributions in Carrie's name to St. Jude Children's Research Hospital or to a local animal shelter.
