She was predeceased by: her parents, Joseph Leto and Frances Leto; her husband Walter Farak; her siblings, Mary Trigilio, Lucy Dwyer, Theresa Zuck, Jean Frontino and Joseph Leto; her grandson Ethan Otto; and her son-in-law Joseph Capone.
She is survived by: her children, Kathryn Capone of Wexford, PA, Natalie Quigley (Dan) of Wattsburg, PA, Walter "Wally" Farak of Gilbert, AZ, Carolyn Zaborowski (John) of Wattsburg, PA, Melissa Otto (Dave) of Mars, PA and Anita Jares (John) of Wind Lake, WI; and her sister Helen Bevilacqua. She is also survived by nine grandchildren; eight great-grandchildren.
In lieu of flowers, memorial contributions can be made to St. Boniface Church.
