She was predeceased by: her brother John Polak. She is survived by: her parents, Robert F. Polak and Dolores E. Polak (Hlivko) of Fairview; her brother Robert L. Douglas (Imelda) of Grapevine, TX; and her nieces, Emily Douglas (Kevin Ross) of Golden, CO and Lindsay Egan (Trevor) of Denver, CO..

She will be missed by all who knew her.”
