He was predeceased by: his parents, John J. Zack and Helen A. Zack (Kovalcik). He is survived by: his wife Carolyn; his children, John M. Zack (Beth), Timothy E. Zack (Meg), Anne C. Parker (Fred) and Daniel C. Zack (Tara); his grandchildren, Marissa, Abby, Emily, Timmy, Nathan, Madison and Allie; and his sister Linda Mish (Thomas). He is also survived by many nieces and nephews.
