He was predeceased by: his parents, Carl Mayhew and Violet Norman Mayhew. He is survived by: his siblings, William Mayhew (Amy) of Erie and Carol Mamrak (George) of Pittsburgh, PA.; his nieces, Crista and Carly Mamrak of Pittsburgh; and his aunts, Lucretia Hummel of Philipsburg, PA and Donna Mayhew of Woodland, PA.
