He was predeceased by: his parent Stacy Chase Jacobs; and his grandparents, Donald, Nadine Chase, Dale J. and Ruth E. Jacobs.
He is survived by: his parent Dean Jacobs; his stepmother Sheryl Jacobs; his sister Katie Bohn (Dann); his nephew Felix; his stepsiblings, Jill Iszkula (Steve) and Derek Connors (Amanda); and also, Ben, Eli and Ivy. He is also survived by many aunts, uncles and cousins.
In lieu of flowers or donations, the family asks that people remember Brian by remaining aware of the mental health struggles that millions of people face every day.
