He was predeceased by: his wife Joan G. Forjohn; and his son Benjamin Forjohn. He is survived by: his sons, Alexander E. Forjohn, Matthew A. Forjohn (Kristi) and Daniel O. Forjohn (Susanna). He is also survived by his 5 grandchildren and 1 great grandchild.
In lieu of flowers contributions in Anthony's name may be made to the Philadelphia ASPCA.
