He is survived by: his mother Elva Jean Petrik (Hartung); his wife Sarah Jane Petrik (McLane); his children, Douglas Allen Petrik (Domenique), Joy Allison Petrik, Kimberly Lynn Forster (Nick) and Leslie Ann Wijeysooriya (Anthony); and his brother Quentin L. Petrik (Sherry).
