He was predeceased by: his parents, Lee C. Cook and Bessie Priest Cook; and his siblings, Sheryl Surat and Lee Joseph Cook. He is survived by: his wife Cindy; his children, Lauren Zajkowski (Robert) of Culpeper, VA, Douglas Cook (Tiffany) of Emporium, Jonathan Cook of Emporium and Jacob Cook of Emporium; his grandchildren, Karen, Payton, Madelyn, Kaison, Cooper, Eli and Quinn; and his parents-in-law, Mary Jane and Fred Guido. He is also survived by several nieces and nephews.
