He was predeceased by: his parents, Harry Forsythe and Stella Forsythe (Flory); his son Chad Forsythe; and his siblings, Judy Layton, Harry and Eddie Forsythe.
He is survived by: his wife Tammy J. Forsythe (Brannon); and his children, Leah Wampler (Christopher) of Elizabeth, David Cochran (Jessica) of Maysville and Matthew Cochran of Elizabeth. He is also survived by eight grandchildren; also, nieces and nephews.
