He was predeceased by: his father LeRoy Moffa. He is survived by: his wife Lisa Dawn Moffa (Aldous); his children, Shane Moffa (Jenna Luczka) of Connellsville, Meghan Moffa, Natalie, Michael and Nicholas Moffa; his siblings, Leigh Ann Moffa of Mt. Vernon and Larry Moffa (Joan) of Greenock; and his stepmother Nancy Moffa (Weber) of Mt. Vernon. He is also survived by many loving nieces and nephews.
The family has requested in lieu of flowers, donations can be made to a Scholarship fund in memory of Leonard Moffa, that is to be established.
