He was predeceased by: his parents, John J. Forish and Irene Scheloske Forish Fenton; and his stepfather Frank Fenton. He is survived by: his wife Elizabeth "Betty" Schwind Forish; his children, Mary Anne Fuhrman (Terry M.), Joanne Vavreck (Robert), John F. Forish (Diane) and Mark A. Forish (Deborah); his grandchildren, Lisa Dugach, Tom, Greg Fuhrman, Kathleen Helman, Dan Vavreck, Julie Aiken, Matthew, Kevin, Andrew and Joseph Forish; and his brother Robert A. Forish (Nancy). He is also survived by several nieces, nephews, and friends.
