He is survived by: his wife Mary Ann (Walsko) of Effort; his sons, John Polak of Effort and Richard Polak (Paula) of Hawthorne, NY.; and his grandchildren, Michael Polak (Allyson), Joseph Polak (Christine), Mark Polak, Kristina Polak and Dianna Polak (Nicholas).
