She was predeceased by: her parents, Robert J. Formic and Donna L. Lucas Formic; and her brother Robert J. Formic, II. She is survived by: her children, Theodore Maleski, II of North East, Jacqueline Maleski of Erie, Michelle Kline of Leesville, LA and Barbara Maleski of Cincinnati, OH; her sisters, Jackie Kaough (Rick) of St. Petersburg, FL and Janice Duke (Dennis) of Erie; her grandchildren, Todd, Sarah, Alexis, Brandon, Nichole, Craig and Shayne; and her former husband Theodore Maleski. She is also survived by many nieces and nephews.
