He is survived by: his parent Helen Foreman (Rhoades); his wife Phyllis Rankin; his children, Janet Geiser (Dennis), Cynthia Dotson (Tom) and Brenda Lasinsky (Dave); his grandchildren, Jennifer Mesoras (Seth), Nathan Foreman, Dainya Williamson (Justin), Josh Foreman, David Eve, Michael Viveros, Scott Shilling (Jocelyn), Brittany Williamson (Tom) and Eden Lasinsky; and his sisters, Barbara Emerson (William) and Iris Wells (Clyde).
In lieu of flowers, donations in Sam's memory may be made to the American Cancer Society.
