He was predeceased by: his parents, Edmund J. Orsulak and Annette M. Orsulak (Krohe); his spouse Stacey Orsulak (Kutz); and his sister-in-law Sherri Grubbs. He is survived by: his children, Alex Orsulak and Jessica Orsulak; his siblings, Karen Krisantz (John), Robert Orsulak (Debora), Larry Orsulak (Kathleen), Louann Hovan (Bob Hall) and Ronald Orsulak (late Tracey); his siblings-in-law, Stephen Kutz (Angela), Sandy Madden (George) and Sara Wesley (Bill); and his parents-in-law, Edward and Berniece Kutz.
