She was predeceased by: her mothers, Catherine Allen Boone and Mary Exeter; and her husband Paul Gene Kostiak. She is survived by: her children, Ann Shaffer (Gary) of Avoca, Paul of Scranton, John "Jack" of Avoca and Cathy Jasnoski of Old Forge; her great grandchildren, Adrianna, Katie, James Klotz, Maiya, Mathias Kostiak Leo Parlopiano, Maurita and John Kostiak; and her daughters-in-law, Donna Kurzweil and Victoria Polito. She is also survived by numerous nieces and nephews.
