He was predeceased by: his parents, Leslie Scott and Betty Burkholder Flack; his brother Randy Flack; his close friend Connie Ghrist; and his grandparents, Eugene, Velma Flack, Randall and Ida Burkholder.
He is survived by: his daughter Emily Taylor (James) of Dunbar; his grandchildren, Kaden, Bailey and Wyatt; his sister Sandra Frazee (Kevin) of Connellsville; his nephew Kevin Frazee, Jr.; and his niece Britni Frazee.
