He is survived by: his children, Scott Johnson (Amy) of DuBois and Leslie Catanzarito (John) of Kentucky; his grandchildren, Haley Jones, Brooke Catanzarito and Olivia Johnson; his stepgrandchildren, Kaitlyn Hoffman and Derek Kutsel; and his sisters, Karen Deasey of DuBois and Linda Sykes of Virginia. He is also survived by five step great-grandchildren.
