He is survived by: his brother Frank Simone; his wife Antoinette Berrios; and his children, Joann Perez (Joseph) of Glendora, CA, Anthony Ortiz (Ann) of Sewell, NJ, John Ortiz (Cindy Getz-Ortiz) and Thomas Ortiz (Diane). He is also survived by eight grandchildren; two great-grandchildren and several nieces and nephews.
