He is survived by: his wife Kathleen; his children, Father Lawrence F. Kozak, Matthew R. (Doriann), Timothy P., Stephen M. and Michael P. (Laureen); and his sisters, Maria Gullo and Stephanie Kozak-Allen.

For the viewing and Funeral Mass, the family has requested that everyone please wear a mask during these uncertain times.
