He was predeceased by: his parents, John W. Kozamchak and Rose L. Kozamchak (Sulkosky); and his sister Rose Copelli. He is survived by: his siblings, Joseph Kozamchak (Connie) of Derry, Mary Crawford (Richard) of Unity Twp, Helen Jones of Greensburg and Shirley Dietrick of Derry. He is also survived by numerous nieces, nephews, and friends.
