She was predeceased by: her parents, Hamilton Lavely and Mary Lavely (Corrigan); and her sister Rose Mary Wasylson. She is survived by: her husband Santo A. "Sam" Formica; her children, Michael (Kimberly), Jeffrey (Susan), Melissa Formica and Christopher (Theresa); her grandchildren, Anthony, Nicholas (Caitlin), Alyssa, John, Sarah (Eric), Joseph (Emily), Samuel, Matthew, Benjamin, Charles (Christopher), Zachary, Jacob, Nathan and Luke; her sister Natalie Raymond (Ralph); and her brother-in-law Robert Wasylson. She is also survived by numerous nieces and nephews.
