He is survived by: his wife Jennifer L. Cook (Fairchild); his sons, Erik Cook and Timothy John Cook Jr.; his mother Helen Cook; and his sisters, Francine Carnevale, Susan Cook and Jackie Louie. He is also survived by many nieces & nephews.
