She was predeceased by: her parents, John Kozak and Ann Kozak (Stephanik); her husband Henry Phillips; her grandson Jerod Shultz; and her siblings, Chuckie, Frank, Joe, Ed, Marian and John.
She is survived by: her siblings, Patty Waters, Joanne Rotola, Len, Bob, Dick, Bill and Tom Kozak. She is also survived by her children.
