He was predeceased by: his parents, Martha Jack (Fox) and Thomas Malvern Jack Sr.. He is survived by: his wife Sherry L. Jack (McKee); his daughter Christine Heeter (Timothy); his grandchildren, Shawn, Emily and Eric Heeter; and his siblings, Robin Hornberger (Ben), Timothy Jack (Crystal) and Jody Jack (Tracy). He is also survived by several nieces and nephews.
In lieu of flowers, memorial contributions can be made to the funeral home to help defray funeral expenses.
