He was predeceased by: his parents, Richard Cook and Charlotte Cook (Skiff); his brother Norman Cook; and his father-in-law Harold Casler. He is survived by: his wife Debra Casler Cook; his daughters, Tricia McChesney (Jason) and Michelle Cook; his brother Mike Cook (Lynn); his mother-in-law Twila Casler; his siblings-in-law, Denny Casler (Laurie), Gary Casler and Shelly Hansen; his granddaughter Kiah Cook; his nieces and nephews, Lola Vincent (Mike), Adrienne Cromwell (Darrin), Erin Boylan (Matt), Kayla Hansen, Shaina Hansen, Zeke, Zack Merritt and Chaz Miller (Tricia); and his great-nieces and great-nephews, Laine, Haylee, Martha Boylan, Charleigh Treacle, Kendall Licea (Chris), Blake Briggs and MJ Boylan.
