He was predeceased by: his parents, Janet II (Neff) and George R. Sample II; and his father-in-law John Burkhart. He is survived by: his wife Debra Sample (Burkhart) of Corry; his children, Sydney and John-George Sample; his mother-in-law Constance "Connie" Burkhart; his siblings, Christine Jordan, George "Scoop" Sample, Susan Sample, Janet Sample, David Sample, Kim Wiler and Patricia "Sam" Stetson; and his siblings-in-law, Tricia Boyer, Thomas Burkhart, Holly DiPierro, Jill Slocum and Christine Bonner. He is also survived by Several nieces, nephews and relatives.
