He was predeceased by: his parents, John Prytulak and Mary Prytulak (Lenko); and his siblings, Donald, Michael Prytulak and Pauline Blair.
He is survived by: his nieces and nephews, Pamela D. Dorazio (Phillip) of Grindstone, Lisa M. Whoric (Barry) of Scottdale, George William Blair, Dr. Leslie M. Blair (Becky) of Parkersburg and Spencer S. Blair (Diane Tatum). He is also survived by numerous grandnieces, grandnephews, great-grandnieces and great-grandnephews.
