He is survived by: his wife Barbara Ann Hazlett (Bushong); his son W. Todd Hazlett (Melissa); his grandchildren, Samuel, Ella, Callum and Kate of Mohnton, PA; his son Dr. Timothy J. Hazlett; his grandchildren, Sage, Avery and Madeleine of Craig, CO; his son Chad R. Hazlett (Tammy); his granddaughter Bridget of Oak Creek, CO.; and his sister Marguerite L. Hazlett of Newark, DE. He is also survived by several nieces and nephews.
