He was predeceased by: his son Jacob "Jake" Mullins. He is survived by: his wife Vera Mullins (Costanzo); his children, Cody Mullins (Sally), James Dale Mullins and Amy Bess; his siblings, Johnny Lucas (Sandi) and Cathy Brumfield (Spud); and his mother-in-law JoHan Costanzo. He is also survived by three grandchildren.

In lieu of flowers you can make donations to the family for a trust to be set up for his grandchildren.
