She is survived by: her children, Michaela Carr (EJ) and John Thomas "JT" Hamilton (John); her parents, Joseph Moffitt and Amelia Moffitt; her siblings, Kristin Jackowski (Michael) and Thomas Moffitt; and her nieces and nephews, Grayson, NavyAnna, Jaynee, Meredith and Walker. She is also survived by many aunts, uncles and cousins.

Burial: Private, In lieu of flowers, contributions can be made for her beautiful children Michaela & JT.
