He was predeceased by: his spouse Judith Ann Babushko; his parent Robert Hendrickson; and his sister Neva Hendrickson O'Brien. He is survived by: his parent Hazel Hendrickson (Smock); his son Forest IV; his wife Lori of Clarks Summit; his siblings, Robert Twp (Maryann), Linda Dougherty (Sam), Hollidaysburg, Laura Lee (Jim Holtz) and Falls; his grandson Forest Hendrickson; and also, Scott (Kate), Philadelphia, Clifford Twp and Clarks Summit.
