He was predeceased by: his parents, William F. Hazelbaker and Phyllis Hazelbaker (Flint); his wife Patricia Doptis Hazelbaker; and his siblings, William, Harold 'Jack', James Hazelbaker and Carolyn Hrivnak.
He is survived by: his daughters, Barbara Phillips (Eugene) of Mandeville, LA and Cheryl Senko (John) of Charleroi; his grandchildren, Patrick Phillips, Christina Morales (Francisco) and Natalie Phillips (Josué Breaux); and his great grandchildren, Emilia, Isabella and Dulce Elena Morales. He is also survived by several nieces and nephews.
