He was predeceased by: his parents, Angelo "Cack" and Mary Margaret Costantino (Archiletti); and his brother Russell "Cack" Costantino. He is survived by: his wife Paula Costantino (Stover); his sons, Jeffrey "Cack" Costantino (Charity) of Fallowfield Twp and Joseph "Cack" Costantino (Heather) of Monongahela; his stepchildren, Nicole Jackowski of Marianna and Matthew Crowe (Lisa) of Greensburg; his grandchildren, Makenley Costantino, Caleb, Jenna Crowe, Max and Megan Shrader; his brother James "Cack" Costantino (Kathryn) of Carroll Twp; his nephew Brendan "Cack" Costantino of Carroll Twp; and his niece Emily "Cack" Costantino of Carroll Twp.
