He is survived by: his spouse Barbara Polchik (Oleschak); his parent Elizabeth Polchik Sr. (Chihuly); his daughters, Theresa Polchik and Barbara Correll (Thomas); his grandchildren, Sean Michael, Taryn Patrice and Tyler Matthew; and his nephew Joseph B. Polchik.
