He was predeceased by: his parents, Michael Mizok and Anna Mizok; and his siblings, Irene Salva, Pauline Guman and Robert.
He is survived by: his daughters, Denise Connor (Thomas) of Clifford Township and Paula Lavelle (William) of Carbondale Township; his grandchildren, Jessica Connor Tonkin (Tom), Ian Connor (Arielle), Tessa Lavelle (Dave Grizzanti), Addie Lavelle (Stanley Kaminsky) and Billy Lavelle; his great grandchildren, Elsie, Coby and Everett; and his siblings, Anna Sue Pezak, Sadie Jesuitt, Mary Ann Jeryga and Mickey. He is also survived by several nieces and nephews.
