He was predeceased by: his parents, Francis Cook and Madeline Kennelly Cook. He is survived by: his wife Carlene F. Holley Cook; his daughters, Lenora Karenbauer (Dave) of East Butler and Melissa Rearick (David) of Butler; and his grandchildren, Madeline Holly Karenbauer and Colton Cook Rearick. He is also survived by nieces and nephews.
