He was predeceased by: his parents, Walter F. Crovak and Malvina M. Crovak (DeNardis). He is survived by: his brother James E. Crovak (Sandra) of Forward Twp; his nephew James W. Crovak (Christina) of Elizabeth Twp; his niece Melissa E. Crovak of Forward Twp; his great-nieces and great-nephews, Matthew Crovak of Blaine Hill and Shelby Crookham of Blaine Hill; and also Levi James Crovak of Blaine Hill.
