He was predeceased by: his parents, Robert Eugene and Mary K. Cook (Hindman); and his brother Herbie Cook. He is survived by: his wife Marjorie "Margie" Ann Smith; his children, Kristen Cook (Kevin Motter), Courtney Forsythe (Michael) and Kimberly Hollobaugh (Jason); his grandchildren, Clayton Cook, Colt Forsythe, Crue Forsythe and Cameron McGrail; and his siblings, Donald Cook (Cindy), Gindy Johnston (Wayne) and Molly.
