He was predeceased by: his parents, Richard Wade Ballentine Sr. and Catherine Haupt Ballentine (Sebring); and his siblings, Robert L. Ballentine Sr. and Carol E. Ballentine. He is survived by: his children, Sherry K. Barton of Brookville, PA, Robert L. Ballentine (Cindy) of Drums, PA, Susan R. Ford of Smyrna, TN and Brenda J. Leuwerke (Randy) of Eldridge, MO; his sister Nancy Marlene Ballentine of Brookville, PA; his grandchildren, Anthony Sherman, Danielle Kahle, Amber Ford, Taylor Ford, Autumn Meinhardt, Dalton Ford, Justin Leuwerke, Kayla Stockard, Jeremy Leuwerke, Ashley Ballentine, Seth Ballentine and Jacque Leuwerke; his nephews, Steven Ballentine and Eric Ballentine; and his niece Carley Perry. He is also survived by fifteen great grandchildren.
