He was predeceased by: his father Garry Cook. He is survived by: his wife Diane Cook (Seitz) of Boyertown; his mother Roberta Cook (Kirkwood); his daughters, Melissa L. Morrison (James) and Crystal L. Schrimer; his grandsons, Patrick J. Morrison, Anthony T. Morrison and Malikia Keeler; and his siblings, Brian Cook (Melanie), Lori Jane Nielsen and Kimberlee Bock (Nicholas Bock).
