He was predeceased by: his parents, Peter Kislak and Stella Kislak (Yankuskie); and his sister Mary Ellen Ward. He is survived by: his pet Dottie; his wife Donna Kislak (Sherrill) of Blairsville, PA; his daughters, Dawn Kislak of Lucerne, PA and Lisa DeWitt (Stacy); his grandchildren, Luke, Elizabeth, Baylee, Jordan, Quinten, Sara, Madison, Skyler, Hunter and Abbigail; his great grandchildren, Isabella and Elianna; and his brother Tim Kislak (Deb).
