He was predeceased by: his parents, Raymond Fails and Clare Fails (McFeaters); his wife Kathleen M. Fails (Dunmire); and his siblings, Phyllis Streams and Bruce Fails.
He is survived by: his children, Barry M. Fails (Jennifer) of Blairsville, PA, Michael J. Fails of Blairsville, PA, Lori L. Jarvie (Brad) of Clymer, PA and Sheri M. Fails of Blairsville, PA; his grandchildren, Ashley Wright, Cameron Kuzemczak, Brett Fails, Madison Fails, Abigail Jarvie, Andy Jarvie and Cole Jarvie; and his great grandchildren, Leo and Stella.
