He is survived by: his children, Jennifer Harrison (Ricky Lakin), Kirk Johns (Lorainne) and Tracy Costo (Todd); his grandchildren, Travis Costo (Jess DeMark), Stephen Rathovich, Brent Harrison, Brent Johns, April Dorsey (Aaron), Harlee Johns and Anthony Johns; and his great grandchildren, Luca Johns and Mia Arlene Costo.
In lieu of flowers, memorial donations in Jack's name may be made to Holy Trinity Lutheran Church, 1098 Third St., Beaver, PA 15009.
