He was predeceased by: his cousin Arianna Jean Ray. He is survived by: his mother Brooke; his husband Kevin M. Hook of Bainbridge; his father Nathan J. Adams of Mount Joy; his stepsisters, Brianna L. and Hailey L. Hook; his half-sister Anastasia J. Adams; his grandparents, Steven E. of Bainbridge, Pamela J. Shearer of Bainbridge, Donald L. of Manheim and Cindy L. Stern of Manheim; his uncles, Keith E. Shearer, Kyle S. Shearer and Jason L. Kaylor; his cousin Khaliya R. Kaylor; and also Deborah J. Henny of Mount Joy.
In lieu of flowers, memorial contributions may be made in Blake's memory to a charity of one's choice.
