He was predeceased by: his parents, David Forman and Mabel Duebler Forman; his wife Dorothy Forman (Hadlock); his son Robert Forman; and his daughters-in-law, Anna Forman and Joan Forman.
He is survived by: his children, George Forman, Jr., Stephen Forman, Sr., Donald Forman, Sr., Kim Boas (Sean), Kenneth Forman and Scott Forman. He is also survived by several grandchildren and great-grandchildren, his brother, and many nieces, nephews, and cousins.
