He was predeceased by: his parents, Henry Jack and Ella Reedey Jack; and his wife Linda A. Jack. He is survived by: his children, Scott Jack (Pam Sprinkle) of Apollo, Michael Jack (Fran) of Brighton Heights and Christina Pinkerton of Apollo; and his brothers, Larry Jack (Goldie) of Ford City and Kenneth Jack of Leechburg. He is also survived by 8 grandchildren, 2 great grandchildren.
