We are sad to announce that on July 13, 2020, at the age of 79, Hazel Hoffman of Apollo, Pennsylvania, born in North Charleroi, Pennsylvania passed away. Family and friends are welcome to send flowers or leave their condolences on this memorial page and share them with the family.

She was predeceased by: her parents, John W. Evans and Mary F. Evans (Boyd); her husband David W. Hoffman; and her siblings, Fern, Violet, Gladys, Ruth, Gerald and John "Clifford".

She is survived by: her son Jonathan Hoffman (Mindy) of Apollo; her stepsons, William Hoffman (Margaret) of Belle Vernon, Robert Hoffman (Margaret) of Smithfield and Paul Hoffman (Candy); her grandchildren, Benjamin Hoffman, Sarah Hoffman and Hannah Hoffman; and her sister Carol Gregg (Gary) of Coal Center.
