We are sad to announce that on September 23, 2018, at the age of 76, Faith Elaine Dvorak (Allison Park, Pennsylvania) passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

She is survived by: her husband Josef Dvorak; her daughters, Carole Lynn Mayberry and Michelle Jo-Ann Sullivan; her son-in-law Richard Sullivan; her grandchildren, Tyler Sousa, Dillon Mayberry, Alison Sullivan and Andrew Sullivan; and her great grandchildren, Isla, Aria and Kenleigh.

Family requests in lieu of flowers, donations be made to Mercy Hospital Neonatal Intensive Care Unit (NICU) (link to follow).
